INFORMATION FROM CITY HALL

We will send the National Health Insurance card.

The National Health Insurance Certificate for 2024 (and Senior Citizen Certificate), which can be used from August, will be sent by registered mail in July.
If you are not at home and are unable to collect your certificate within the post office's storage period, you can collect it from the city hall. Please contact the relevant department.
Please note that due to a change in the law, current insurance cards will no longer be issued after December 2, 2024, but the insurance card we are sending you this time can continue to be used until its expiration date.

[What to bring if you are coming to collect]
Something that can verify the visitor's identity (My Number card, driver's license, passport, or something with a photo)

※ If someone from another household is collecting, a letter of attorney from the head of the household is required.

[Notification must be made within 14 days in the following cases]
Notification of loss of eligibility for National Health Insurance
[When you join your employer's health insurance, or are certified as a dependent]
Notification of enrollment in National Health Insurance
When you lose eligibility for your employer's health insurance, or are no longer certified as a dependent
